Choice Health professionals come from diverse backgrounds and experiences to care for our patients. Staff members and services include:
RN Case Managers
A RN case manager coordinates all aspects of care and evaluates patient needs. He or she also implements the physician treatment plan and monitors the care and condition of each patient.
RNs & LPNs
RNs and LPNs are licensed caregivers that can provide the following services:
- Skilled assessment & observation including vital signs
- Injections
- I.V.
- Wound and bedsore care; includes using specialty products such as Wound Vac
- Post-operative wound care and instruction
- Patient and caregiver teaching
- Medication management and instruction
- Foley catheter insertion and care
- Bladder instillation
- Bladder and bowel training
- Venipunctures
- Diabetic care and nutrition
- Restorative nursing
- Psychiatric nursing
- Ostomy care
Physical Therapy
- Assessment, therapeutic exercises and training
- Establish home exercise program
- Gait training
- Cardiopulmonary training and energy conservation techniques
- Work with large muscle groups
- Orthopedic and post joint replacement therapy
Occupational Therapy
- Assessment
- Teaching self care skills
- Trains patient how to use medical and prosthetic devices
- Works with motor skills
- Provides orthopedic and joint replacement therapy
Speech Therapy
- Assessment
- Dysphagia treatment
- Speech disorder
- Aural rehabilitation
- Voice disorder treatment
- Non-oral communications
Certified Home Health Aides
Choice
home health aides assist with bathing, personal care, hygiene,
ambulation and other activities of daily living. Aides
can also prepare light meals and assist with feeding as
needed. Choice aides can help maintain a safe and
clean home environment. They can also observe the patient
and report the patient’s condition or relate questions
to a nurse.
Medical Social Workers
Medical social
workers provide assessment, community resource & referral
service and an understanding of significant emotional and social
factors related to health problems.
Registered Dietitians
Registered dietitians provide nutrition therapy to Choice patients as needed.
We can also coordinate your medical equipment needs, provide referrals to assisted living facilities or nursing homes, assist with the Medicaid waiver Program and help find a private duty nurse.
A Choice nurse is on call 24 hours a day, seven days a week. |
