Field and Support Staff
Choice Home Health and Choice Hospice currently have close to 50 employees, including RNs; LPNs; CHHAs; Physical, Occupational, and Speech Therapists; Registered Dietitians; MSW; Chaplains and support staff. Collectively, they have more than 200 years of home health and hospice experience.

Management and Clinical Team
Umi K. Chahal, M.S., MBA, RD/LD , serves as chief executive officer of Choice Home Health, Inc., and Choice Hospice, Inc. For the past 13 years of her 34-year tenure in the health care field, Umi has worked in the home health and hospice care industry. She is a registered dietitian with extensive experience in health care administration, home health and hospice services, rules and regulations. Prior to her home health and hospice experience, Umi worked in a hospital setting as a clinician, manager and administrator. Bhajee S. Chahal, B.A., serves as president of Choice Home Health, Inc., and Choice Hospice, Inc. Bhajee has spent 12 years in the home health services and hospice care industry. In addition, Bhajee has 34 years of business executive experience and has excellent marketing and inter-personal skills.

Keli Ingram, R.N., director of clinical services, is a 14-year home health care and three year hospice veteran, having served in a variety of capacities, including clinician, quality improvement director, educator and director. She is experienced in policy and procedure development and process implementation for both large and small agencies. Keli has worked closely with the Oklahoma Foundation for Medical Quality in analyzing statistics and developing improvement plans to ensure the agency’s quality. Susan Osbourne, R.N., serves as the Hospice Coordinator coordinator and has more than 27 years experience as a nurse with more than seventeen years in home care and hospice. Prior to this she was an ER Nurse. She is certified as a hospice & home health administrator

June DeShazer R.N., serves as clinical coordinator and has more than 20 years experience as a nurse, with more than 15 years in home care and hospice. With excellent clinical and I.V. skills, June does an excellent job of coordinating care. She has served in multiple roles within home care and hospice from direct patient care to management.

Susan Crosby R.N., B.S.N, serves as clinical coordinator and has more than 10 years experience as a nurse. With a background in cardiac care and medicaid advantage program, she has excellent clinical and management skills.

Amanda Allen R.N., B.S.N, serves as the quality assurance & improvement coordinator and has more than 9 years of health care experience. Her background includes Assisted Living management, Quality Assurance, and ER nursing.

Sumeet Chahal, B.S., (Computer Engineering), serves as CIO/COO and has been in the home care and hospice field for three years. His technological skills have been instrumental in moving the agency toward State of the Art Point of Care technology as well as implementing both home telemonitoring and digital communications.
